If you only notice it while in the cab driving, you should check the steering shaft seal where it passes through the firewall. If it's cracked or torn it can allow excessive engine noise to enter the cab.

Good point - More info on that in this sticky thread for OP: https://www.tundras.com/threads/diy-steering-shaft-seal.48664/

Were the '03 also impacted by exhaust manifolds cracks like the earlier years?

When you replaced B1S2, did you clean the exhaust side mating service and did you use a new gasket ?
From the looks of the B1S2 sensor you posted above and the continuous lean condition(less fuel/more air) in graph when reving, it causes me to suspect air being drawn in due to bad mating service contact. Bottom of that sensor if indeed new, has collected a lot of gunk/gasket flakes..

OK, update. Turns out the brand new O2 sensor from Napa was defective. They're not giving my $ back but at least I avoided a trip to a shop. Just waiting for the evaporated monitor to complete and I'm ready for inspection.

I attempted to remove B1S1 today but the 7/8" O2 sensor socket I bought fit very loose. So I aborted on that task. Probably won't bother with it since truck probably won't pass inspection next year.

Lesson learned, just because it's new doesn't mean it's not defective.

Awesome, and well ... this just speaks to the narrative above about avoiding certain parts. I'm not gonna be that asshole who says "all OEM all the time", I know there's a time and place for that stuff.

Curious to see what outputs look like when you get your new Denso downstream replacement in .... and that's kinda bullshit NAPA won't refund you. I'd expect them to do one better. You have the OBD output to prove it's acting erratically. I'm sure they only want to offer replacement though, not refund.

Honestly, knock on wood ... I've yet to see a single Denso part purchased from a legitimate source (i.e. not scAmazon/fleaBay/WallyWorld online) that's been defective out of the box. I've seen plenty of big-box auto parts stores parts defective, and numerous people on this forum and others I've witnessed getting DOA parts, spending ample time replacing, and with no recourse against the store or parts manuf'er. It's honestly a crock of shit that WE the customers get to be their QA/QC team, and we don't get paid a dime for it.

I'll do a screen shot of the B1S2 & B2S2 output. I'll try to record a minute or so if that's possible. I will say this. I was using a dongle with torque pro. I purchased an add on the enabled me to view sensor outputs. It was invaluable with helping to diagnose. Being able see what sensor were doing was extremely helpful.

Thanks for all the help and suggestions

I'll also say it's remotely possible I messed up the sensor some how but, I doubt it. I was watching a video on changing an 02 sensor yesterday. The woman mentioned that the sensor comes in a sealed bag. It dawned on me the first sensor I bought wasn't bagged.
