I can't for the life of me level two two exhaust tips on the TRD dual exhaust. I tighten up bolts and the drivers side is too high. I put the used hanger on the driver's side and it doesn't help.

    Any thoughts?

Few things to try others have had success with:
    New rubber hanger
Hose clamp around the used rubber hanger to tighten it up
I used a jack stand during install to hold up the pipe in place while i tightened the bolts

I tried jacking up the low one quite a bit before tightening. Honestly the low one is where I think the tip should be.

The driver's side is too close the paint/bumper.

I'm wondering the driver's lean is contributing in some weird way.

I have the same problem (driver's side needs to come down about 1 -1.5"). I'm going to take a look at it again this afternoon. I'll let you know if I solve it. Two suggestions so far are to loosen the 3 bolts between the tailpipe and muffler section and rotate and re-tighten. I might use a jack stand or something similar to hold the tailpipe at the correct height. Other option is to lower the drivers side hangar where it's bolted in place by using washers or some type of spacer to force the pipe lower - this would probably require a new longer bolt.

A friend held up the passenger side while I tightened the nuts. Both sides are level.

I used a floor jack to "twist" the whole system, so the passenger side would be higher, driver's side lower. I first did it to the muffler, when I tightened the bolts to the cat. Then I jacked up the passenger tip.

I have two new and one used rubber hanger to use on the rear two hanger locations. No combination seems to work.

I'm sure I am missing something simple somewhere.

I definitely have a driver's side lean, something I am thinking should be fixed with some shims. But, I don't see it directly fixing an exhaust that is attached to the body. The tips compared to the axle seem pretty freakin' close.

Keep in mind the install instructions from Toyota stated, "While tightening the bolts, set a gap of 30±5 mm between the exhaust tip and the rear side panel".
    That's only about 1.2".
I was able to get them level with the jack stand method.

more than I can list here
Drill the holes bigger on the flanges. Or even slot them.

Big shout out to Keith @dirtydeeds . While I was once a customer, I didn't buy this exhaust from him. Despite that, he still returned my phone call and we discussed the fix above... Haven't dived into it yet, but the fix makes perfect sense in my head!
