I've got an '03 Tundra Access Cab. Recently I started having this problem where the doors will lock with the fob, but will not unlock with the fob. I thought maybe this is a common problem and somebody knows how to diagnose and fix it. And maybe some tricks and tips. I've done a little research, but I want to hopefully be confident what the problem is before I start pulling doors apart.

I had that happen before and I had to use it 2 ft away for it to work. A new battery fixed that issue.

Yep, new battery. If that doesn't work......new fob.

I found (probably by a post on this board) that if I hold the fob against my cheek that the door will open when it otherwise won't.

I replaced the battery right away and tried the spare fob. Same results. That’s what made me think the problem is in the door.
How do you get a new fob?

Not sure if this would apply to this particular year of vehicle, but sometimes when programming a fob there is a sequence of things you have to follow to get the fob paired to that particular vehicle. Maybe see if there is a sequence with your vehicle and try it with your current fobs.

Before getting any further, can you please post a picture of your fob so we know which system we're advising you on? RS3000, RS3200, afermarket?

Looks like you're using the RS3200 TVIP security on the truck. Programming instructions are here for Reg Cab, Acc Cab, and some Db Cab.

I'd buy a different remote and see if that gets you sorted; if not, you've now got a spare remote. Bill just gave the link, you basically need the key fob with FCC ID "BAB237131-056". Searching for the phrase BAB237131-056 anywhere on the internet will net you a replacement. The hardest part is programming it - it normally takes me 2-3 tries before I get the order/timing of steps correct, and I've done it half a dozen times on this truck already. Doesn't seem to get any easier with repitition, so keep trying the exact instructions in the linked document.
