Still on your original battery? They've been known to go bad after about 3 years.

You could also be affected by the fuel pump recall which can make your truck stumble on startup.

My truck had a series of odd issues when my battery was dying off. My seats would routinely try to pin me to the wheel, door locks would act up when i was driving and my temp display was always off. A battery health test likely doesn't cost much if anything.

Watch these videos, this guy had to have his fuel pump replaced. Does his experience match yours?

Not like that mine cranks rpm drop to 400 and then runs fine after a couple of seconds

Sounds like same issue I had with my 2016 with 72k miles about a month ago. Look for VIN match in "Additional Information" section. I had to argue with local dealer, but after showing this, all was taken care of by Toyota. They kept telling me that "There are no recalls for your truck", I kept telling them, "This is a TSB, not a recall". Good Luck!

As @16CMTXED said, could be a flex-fuel issue if your truck is a FFV. The problem is, the alcohol density sensor gets out of whack and thinks you’re running too much ethanol (even in my case, have never run a drop of E-85) when it’s cold. This causes the truck to run a different firing sequence and leads to weird fuel trims and extended cranking. Reflashed my ECM twice to no avail, eventually the fuel pump failed due to all the cranking. Finally, my problems have gone away when I got the supposed newer style fuel pump with those recalls from 2019 and reflashed the ECM again for good measure. Truck starts up first try on negative fahrenheit days here in Colorado.
