I got LED bulbs for my fogs on my 2018 but they spread the light a bit too much. Should I get the whole unit to make it more focused and if so any suggestions?

Yes. Either find some for sale in the marketplace on here or go Morimoto, Baja designs, Rigid, etc. Not knowing your budge it’s hard to make a good suggestion.

TRD Air Filters, Front door windows tinted to match the rears. Wheel locks, Talons Garage cat shield, Tyger Automotive soft tonneau cover, WeatherTech floor mats with Tuff Skinz Custom Emblem kit, Toyota OEM Emblem Black Out kit. AVS vent covers on all four doors, Red Tundra letters on front bumper & TRD highlighted in Red on front grille. Tailgate Tundra Letters in Black.
Good results with Morimoto 2.0’s here for my fogs.

Just put a set in my wife’s Sienna for the low beams. Super bright

Are stock 2018 led fogs on point with morimotos? I have a 2014 and looking to upgrade. They are around the same price. Any issues with fit?

HID retrofit headlights from Lightwerkz, Morimoto LED fog lamps from Lightwerkz. JL audio upgrade from Tech12Volts, Powdercoated factory black wheels local, ESP rear storage trays under seat, bed cover by BakFlip, bed liner, console safe, LED light bar by Back Country. Weathertech floor liners and ventshades. TRD rear sway bar. TRD Pro exhaust, TRD Skid plate, Fortin EvoOne Remote start from Anderson Electronics.
I chose Morimoto when I did my retrofit, threw them in as a package deal. Light is focused pretty well, but I set them low. I like them.

I put some Lasfit Leds in mine and they work great. They light up the road nicely directly in front of the truck which is what I think you want out of a fog light.
