I had the 46 gal Transfer Flow on my old tundra…even that one poked out below the frame rails.

It was nice not having to stop at every gas station, but don’t know that I’d do it on my current Tindra

 

If it were me I would lift the bed off (cherry picker or 2 post lift if you have one), do the pump replace, then use the opportunity to fluid film the hell out of the back frame and underside of the bed. I've never had to do it on my tundra, but the last 3 fords I had all got it done.
