Would like to purchase and install lightbar under tailgate but the access hole for the spare tire kinda prevents this. I had the same issue with my '23 Tacoma. Anybody have success with purchasing one or a workaround for installation/mounting? Thanks for any info!

The Toyota OEM Rear Light Bar Kit solves this.

I ended up with a generic Putco 9201048 light bar. It's narrow and attaches via clips. If you need access to the jack hole, you can unclip the light bar out of the way. I tried the OPT7 off Amazon, it was wider than I wanted and not as bright as the putco. Either can be easily connected with a adapter harness.
