I'm in the process of doing some deferred maintenance before I repair a rusted bed and while nothing else has been too egregious I found these yesterday.

    Forward Hitch Bolt/Nut

    Middle Hitch Bolt/Nut

    Bolt Location

My initial thought was to just cut off what's left of the nut and put in a new weld nut but I'm second guessing that now given the amount of swelling around the forward nut.

I would say cutting the nuts off is a given, but you are past welding one back on. You need to clean that up and weld some plate on first.

I'd just drill some new holes and add some more bolts to it.
