Lean Bank 1 and 2 at tripped idle only

    2008 Tundra 5.7, Lean Bank 1 and 2 at tripped idle only. 164k

    Hi, long time reader and user of many threads here, but now I have to ask.

    Code reader shows bank 1 and 2 lean, occurs at stop lights or while backing up slowly etc. Did a smoke test on intake etc, found a small leak due to not tight enough intake bolt. Code still came on. O2 sensors appear to be within range at idle and at higher rpm. MAF gm/s are within specs at idle and at same rpm. Plugs were changed at about 120k, filter clean and good.

    I had taken it to toyota once in it's life recently and they told me bad fuel. They dropped and drained tank, said it had a lot of ethanol at the bottom and to use premium.

    My next venture is fuel pressure test? Anyone have any other advice?

    Start with fuel pressure. PCV? maybe leaking internally and smoke didn't show during the smoke test?

    Pressure test failed. Got 30psi in start up then idled down to 20psi at 1000rpm.
