Question about Toyota form

    IMG_0606.jpg So I’ve spoken on the forum a few times about the key fob issue I’ve been having with my 23, key fob has to be placed on start button to crank truck. I’m in the process of taking the truck again to the dealership for the 4th time and was curious about a form I’ve been asked to fill out for a Southeast Toyota Field Technician. Does anyone know what the deal is with this form since this is the fourth visit? So far they’ve replaced the fob battery, reprogrammed software and recently replaced a key fob. I would have thought the form would have been a little sooner in this process ? Any thoughts
