I love my 2002 Tundra SR5 V-6, but with it passing 200k a while ago i am wondering if it would be appropriate to go a step up in oil to 10-40. The oil gauge always looks to be on the low side when it is idling hot. 5-30 seems kind of thin to me. On an engine with this many miles will the thicker oil cause problems?

No, keep the recommended oil. 5w30 is great.
Oil pressure varies with RPM, hot idle is low and as long as pressure increases with RPM you should be good.

I’m willing to bet if your 200k mile truck was properly cared for, there’s minimal internal engine wear and 5w30 is still appropriate. Leaky seals may benefit from a higher mileage oil formulation that many offer.
