This has been around for a while. I believe it started with Toyota (could be wrong) a decade ago? maybe more? where a floor mat had creeped forward and got lodged in the pedals and caused a high-profile collision. Since then, we have had "floor mat inspections." It is also why most - if not all - new vehicles have floor mats that clip in to the floor carpet to keep them from moving.

Yup, I think it jammed the accelerator of a Prius driver on the interstate

I had these old weaved fabric Matt’s in my Honda Prelude. They got wet one winter and froze. I jumped. In next morning and same thing, jammed the accelerator and I couldn’t pull the Matt back so I just turned the car off and slowed it that way

thew those Matt’s away the same day

The scenario that caused all the hullabaloo was a Lexus loaner car with universal sized aftermarket floor mats that got lodged in the accelerator pedal causing the car to accelerate to high speed (~120 mph), the ultimate issue that caused the crash was that the Lexus had a push to start button and the driver did not know how to turn the car off when this happened.
