Rear Parking Sensor Loose

Just recently I noticed that the rear parking sensors are not flush with the bumper. The bottom of the sensor is protruding past the bumper and the passenger side sensor caved in. From time to time time, I'll get a warning to clean the rear parking sensor. Has anyone had this problem before? Is there a bracket that the sensor should be attached to?
