Had the exact problems; Brake Controller Error, 4LO 4HI both flashing, Trac Control light on, ABS light on, Check Bling Spot Monitor light on. Also, BSM was inop and 4 Hi would not engage. Turned out to be the left rear wheel bearing. The tech told me that as the bearing was starting to fail it was kicking out small metal particles which were fouling the magnetic signal between the wheel speed sensor and wheel hub. Makes sense since the truck sees that wheel doing something totally different that the other three.

Before taking it in I did all the recommended checks: all wheels for any lateral movement or sound with a stethoscope while turning by hand, all that was negative. Also, no fault codes were recorded. Tried removing negative battery terminal for 30 minutes and clearing codes even though none were recorded, battery voltage and alternator output were all good, nothing helped. 189,000 miles and of course this all happened while I was 400 miles from home.

I had this problem back in Sept 2022. Went away after turning truck off. Just came back on yesterday (1/9/23) and won't go away. Having a mechanic do my trans and radiator flush tomorrow and going to have him diagnose the issue, which I've told him is likely the rear wheel bearing/axle seal, from what I've read. I have an appointment next week with dealer since I have an extended warranty and it's covered. Got 10k more miles (and until 2026) on the warranty so hopefully if it needs anything else, it happens within the next year...or not at all!

I ended up taking my truck to my mechanic to do the trans oil (drain/fill) and radiator flush, as I had to leave for an out of state hunting trip. Had him diagnose the issue. Left rear wheel axle seal and bearings. I have an extended warranty up to 100k, and my truck is currently at 90k. Called Toyota Certified, and it's covered. Dropped off at dealer this morning. They'll confirm what the problem is (for $160 diagnostic cost that is not charged if repair is under warranty), confirm it's covered and get it done. I gave them the paperwork from my mechanic, who they confirmed is a very good nearby/local shop, and then the service tech said, "legally, I have to tell you all this...." I replied, "that's fine...I'm an attorney...I hope it's done by Friday"

Keep you all posted, but I fully anticipate the dealer taking care of this per the warranty.

Im having this issue. Goes off when you turn the truck off, then after you start driving again, comes on.

I dropped the truck off at Toyota in Alexandria, La after Thanksgiving and picked it up last week with no work being done. They said that they were waiting for parts, which they initially said was a two week wait. Then after I checked out and was told I owed nothing, they called and wanted to charge me a $250 "restocking fee" for parts that they never received. I told them that I wasn't paying that and they voided the charges. Toyota also wanted $3500 to fix it.

Crawled under the truck and this is the right rear. There was also oil on the outside of my wheel. Taking it to a different mechanic shop next week that said that they can have it fixed within a week.

I remember reading somewhere that oil pushing out of the side like that can be due to a clogged breather valve.

Just picked up my truck yesterday from dealer. L axle seal, bearings and speed sensor. While the sensor could be cleaned, the tech told the service advisor it likely will get tripped again and be back, so they authorized the sensor replacement too. Covered under the extended warranty, 100k miles/2026.

FYI jdg1982, the total cost had I paid for it was $1500 from the dealer. If you want a copy to show your dealer, PM me. We can figure out a way I can send you a copy. $3500 is a rip off, even if they were doing both sides. That's bull shit

drove my truck to AZ (from So Cal) for a hunting trip with the light on entire trip. 1300 miles. No issue. Had it repaired as soon as I got back. It’s a leaking rear axle seal and bearing tripping your speed sensor.

Actually I heard you could have issues putting it into 4wd. But that’s about it

I'm in this boat too. Found a parts list from another thread. Ordering the wheel speed sensor, new abs sensor wiring harness, bearing hub assembly, bearing seal, and a few other odds and ends that @Track Day posted from his dealer invoice. I isolated the bad sensor with my obd2 scanner and when I pulled the sensor it had gear oil on it. I know the sensor I have may still work if cleaned but I'm just replacing it anyhow. Harness has a few nicks in it so that's why I'm replacing it as well. Total cost in parts of everything is just over $700. Once all the parts are here I'll install and update. If it looks like it's just the bearing seal I'll stop there and return the bearing hub assembly which would save me $323. I currently don't have play in the wheel and I'm thinking I might not even need it, but ordered it to have it on hand.

As @kevine0001 already noted, I too have driven over 700 miles on mine due to where I was when my lights started this process. No problems shifting or driving it. It will go into 4hi and 4lo if I switch the actuator before I start driving once I start the truck. Once I drive approx 50 yards the 4hi 4lo lights flash and at that point the actuator won't engage 4x.

This is what my sensor looked like when I pulled it.

Had similar issue on 2012 Tundra 4x4 with 200K miles. Ran my scanner with ABS code abilities on the vehicle (Innova 5610) and it said Left Rear wheel sensor bad. Funny thing is the live data showed the sensor was correctly reading wheel speed. Did not detect any wiring damage so I replaced the sensor for about $30. Problem solved.
