After a 1000+mile trip found this piece of loose trim on my front drivers side door.

    From factory TRD off road package with 275/65/r18 TRD wheels;
    what is this adhesive (in pic), where do I get it, and how do I apply it for a factory finish. thanks.

    You can get 3M double sided trim tape at any auto parts store that will suffice. I had the same issue and used the 3M tape with some clamps for a couple hours and it has held.

    Should I remove the pad shown, scrape underside surface clean, and then apply 3m or apply 3m to it as is?

    I'd try to clean it as much as possible before applying.
