2001 Tundra V6

This is the first vehicle forum I've ever joined, plenty of motorcycle forums, so I'll ask my first question.

2001 standard cab V6 (145K) has a first gear only, noticeable whining noise for a few years now.
I've replaced the fluid (just a pan drop) a few times over the years with Amsoil.
Any thoughts on what this would be?
The fluid still looks and smells like new.
