Bad parts, bad craftsmanship, or both.

Their failure to inspect for wear on axle sealing surface resulted in the axle being replaced the second time, and they still couldn't get it right. Go to another shop and specify that ONLY TOYOTA PARTS be used. Also specify that you want the old parts returned in the Toyota boxes or packaging. If they grumble, walk, and keep looking for another shop.
