Startup noise - chain tensioner?

    2010 Tundra 5.7L 4x4
    over the last week my 5.7 tundra has developed a startup rattle.
    Sounds like a loose timing chain tensioner - the rattle goes away after about 1-2 seconds.

    I bought it 7 weeks ago - and havent had any problems until now.
    toyota records show it had a very expensive service done at 100k miles - I suspect it was cam towers because they are spotless at 135k miles.

    Could this only be the chain tensioner or could something else cause it?
    how hard is it to change the tensioner?
