Seized tensioner pulley

I just changed out a stuck tensioner pulley on my 2003 v8. Used the same bolt and outside washer. New pulley will not move at all. Should there be a bushing on either side?
The tension is great so I hate to change out the entire assembly?

Thank you very much for the prompt reply. The diagram does help a bit. I removed the washer on the old pulley (it is large and seats on the pulley rather than the bearing; whereas it did seat on the bearing of the old pulley) and viola' the pulley moves. More noise now than what got this started. From the diagram it looks like there is a washer/bushing on the engine side of the pulley so I guess that will be my next move.

Are you sure the noise isn't coming from another pulley?

 

Well I checked all the pulleys when I had the S belt off and they all seem to be in pretty good shape. I need to do a water test again as the belt has gone several hundred miles slipping over the stuck pulley and the smooth side is quite glazed. A new belt is probably in order.
