How cold is cold? Mine sounds angry starting when it's -20 but I figure that's pretty normal.

High pitch might be the serpentine belt and or tensioner. At that mileage wouldn't hurt to do both if not done recently.

Take the belt off and turn pulleys by hand to try and find any bad bearings. You can also spray some starting fluid around in the engine bay and see if you notice any changes in idle to find intake or vacuum leak. Watch some YouTube and see some examples.

I’d also recommend unhooking the battery for a bit to force a reset on the idle parameters.

Have the spark plugs ever been changed? CEL on?

Is it fan roar, with an initial whine, I think its the air injection, when its cold? I would say its normal, unless its an actual squeal, then its something else.

LOL, yes I know this, but with 3 posts in and asking this type of question, I no longer assume people know what SAIS is...

If its an electronic type whine best way I can describe it. its good to go, if its a squeal I'd have it checked out.

Others have recorded their noises on a YouTube video and posted.
Folks are suggesting what it could be. Without a better description or at least audio, you need to have it checked out for a better diagnosis.
