Shiiit, if I had access to a welder, I'd have tacked two little dots on both LBJ nuts on both of my trucks.

In easy to get to places with an angle grinder of course.

I think someone already mentioned this but, it seems like there is an issue with the taper. The nut should pull the ball joint down into the hole and seat the taper. If, for example, the hole in the arm is worn and there is not a good fit between the taper of the arm and the joint it will allow things to turn. Or, if things bottom out before the taper is seated, it will let things move.

I have seen it on other cars before.

    Mevotech are crap. Get OEM.

04-06 wouldn't bolt to his spindle, it's larger iirc

^ this. I've seen several aftermarket joints (tie rods/ball joints) that has the tapered area longer than OEM, so it sticks through and never seats properly.

I'd back that nut off and see if maybe the taper made it past the LCA hole and you could never properly tighten it.

Little update, I've taken alb1ks advice and pulled it apart and applied some anti-seize to the taper. Then cranked it down quite a bit, so much so the cotter pin hole was beyond the castle nut. So as a temporary fix I added a washer under the castle nut. I'm going to keep a close eye on it to make sure it doesn't back out.
Unfortunately the popping noise still persists. I have figured out how to recreate it though. Driving forward and fully locked to the left it pops once and wont pop again until driving in reverse fully locked to the right. This repeats etc. Sounds like a CV to me but those were also replaced about two years ago. Also the LBJ nut has not backed off since recreating the popping noise several times.
