    This will be contacting the ground as a garden surround, and it's not a garden for edible plants, so no concern. I live in a heavily wooded area with a high termite burden, so I feel like it's the best option for this.

    I washed a shit ton of my neighbors blood off of my front porch yesterday.

    My neighbor and good friend does dialysis and about a month ago he called me and said he needed help. I ran over there and he was sitting on his bed and blood was gushing out of his arm. We were able to keep pressure on it and I called 911. He went to the ER, got a stitch or two and was on his merry way.

    Yesterday afternoon, I was in the back of the house reading a photography magazine and the doorbell rang. I open the door and he's sitting on our bench with blood just pouring out of his arm and he's trying to keep pressure on it. There was blood everywhere. He said "Can you call 911?" I'm calling 911, yelling to my wife to grab puppy pads and asking him what happened. Meanwhile, his dog his circling our porch, doing dog stuff.

    My wife brings out the puppy pads and he says something about taking off his bandaid and then he slumps back on our bench, falls sideways and starts snoring. I'm still trying to tell 911 to get somebody over here. I was able to grab him and sit him up on the porch with my knees supporting his back and I had my right arm crossing his body and squeezing his arm as hard as I can to stop the bleeding. My wife took the phone and is telling them what was going on. It seemed like he seized at some point and I'm yelling at him to wake the hell up.

    He's in and out but he can talk to me. 10 minutes had gone by and I'm yelling to my wife "Where the fuck is the ambulance?!" And then I said "Can they send out a firefighter or something?!"

    Fortunately an officer showed up and took over with the pressure. I was still holding him up. Neighbor was a little more awake and could tell the officer who he was and all that shit. EMS showed up, and while two were stabilizing him, one looked at all of the blood on the porch and asked if that was all of it because he needed to estimate blood loss. We took him to the neighbors house and it looks like somebody was murdered there and dragged to our place. Big old blood trail. He estimated he lost over a liter.

    They stabilized him at our little hospital and then airlifted him to a hospital about an hour away, in our old hometown. Drove out there last night with my wife and talked to him at about 11pm and he was doing great - his usual smartass self. He was removing a band-aid and blood just started pouring. He said he picked up his phone and yelled "Joel, help!" and then realized it didn't work that way. Then he somehow walked over to our house.

    He's not doing so great today, they thought he may have had a stroke at 3am but they've found no signs of clots or anything so now were are hoping it's an electrolyte issue and after he gets his dialysis today, it'll kind of reset him.

    Anyways, I washed a shitload of blood off of my porch yesterday.

    dang that sucks. Glad you stayed somewhat calm (hopefully) and helped. Next time lay him down flat and just elevate his arm while keeping pressure on the wound.

    It's scary how long response times are where I live but when you start packing in apartment buildings and shoe horning houses everywhere you can you have more people using 911 services. It hurts even worse when they don't want to pay more taxes to grow those same 911 services that they demand...

    Good job helping your neighbor.

    People bleed out from dialysis fistulas as they are arterial (they are also venous), and I have seen one patient exsanguinate, (bleed to death) before we could even get there, but he was also alone. Your neighbor, and doctors will hopefully figure out why this keeps happening, whether he is way to anti coagulated, they are not leaving pressure on long enough at the dialysis center, or maybe he is doing strenuous activity that is leading to the loss of his clot. I am sure now he realized that he could die from this and maybe he will figure out a solution, along with his doctors, before he collapses and dies before he can get help.

    He should also come up with a plan in case it happens again, so that if you are not home and he is alone, he can do something to slow the bleeding at least. It's hard to stop an arterial bleed when you are by yourself and it's your own arm. They have special clamps that hold pressure at the dialysis center, sounds like they need to let him wear them home, or at least have it available at home.

    Scary stuff....

    A quick clot bandage would be something he should maybe have. You can buy them online.
    Or an Israeli bandage to help with pressure.
    Even a CAT would be a help in a life threatening situationlike that. I keep one in my car, house, boat and backpack. Used one from my car when I was at work one day when a guy got his leg with a chainsaw.
    I'll probably never use one again but they are cheap and can save a life.

    I was not offering this type of advice because this involves a dialysis fistula, basically that guys lifeline so to speak.

    the person needs to consult with their nephrologist or vascular surgeon before using any of that stuff, especially as a lay person.

    I don’t think think quick clot would help because the bleeding is inside and it’s a needle size hole it’s coming from, that is made for open wounds.

    If he dies he won't need dialysis access. End of story. Hemodialysis is needed every other day at least. Sometimes every 3rd day. If he saved his life and gets to medical treatment a new fistula can be placed. A tunneled IJ catheter can be used until a new fistula can be created.

    We get patients all the time who need emergency dialysis for various reasons, patients who may not even have chronic renal disease. And you're right, we can get a temporary tunneled IJ into them in 15 minutes when needed. I would hate to be the guy who tells his family that I let him die because I didn't wanna disturb his fistula.

    Yup. And a fistula can take 6 weeks to mature and be ready to use. We place fistulas and IJ cahts all the time. I am wondering if this guy had a graft or vein fistula. We tend to see more bleeding issues with the grafts vs veins.

    My point wasn’t to say just let him die, my point was to address the fact that he needs to speak with his physician FIRST while in the hospital and when he is feeling better, if he does get better, to figure out the best course of action if this were to happen again, not that we should watch him die in order to save his dialysis fistula.

    maybe I worded it wrong or it was interpreted wrong, either way I wasn’t at all saying let the guy bleed to death , I was saying for liability reason on the part of the Good Samaritan neighbor ( yes, I realize the Good Samaritan law exists), and for the safety of this patient it should be discussed with a physician who best understands fistulas and shunts what the best and safest way to hopefully either nip this problem in the bud or how to stop the bleeding in future instances.

    btw it is not always easy to gain access again once one is lost, especially in long term patients who already have a bad shunt or fistula in the other arm etc. someone posted on here like some magical fairy will come and place a new perfect LONG TERM access point for this patient, and I have news for you, that doesn’t always happen. You guys are thinking about right now and not the fact that this patient may need another shunt or fistula five years from now and another new one in ten years from now etc. they don’t last forever!

    I always keep a small first aid kit in all the vehicles and a larger one at home, but i'd like to build all those out a little further.

    Ouside of a normal first aid kit, what do you guys recommend keeping on hand? Sean mentioned some quick clot, any specific to look for? I'm not going to be setting up a hospital in my living room or anything, but with all the home improvement projects, drinking (not at the same time of course), pew pew'ing in the woods, it would be good to have a bit more on hand.

    Bleeding out like that we don't care what happens to the fistula. We worry about the patients life(as evidence by the amount of blood loss and the fact the patient is passing out). A new fistula is available depending on where the first one was placed. Fistulas don't last forever...but they can. All sorts of complications can arise to new and old fistulas. Pseudo aneurysm, collateral arterials, ect. But there are multiple areas a fistula can be placed. It is a way to prolong life. Fistulas can be placed in the forearm, upper arm, legs, ect. And even if veins are too small, a graft can be placed instead or even the vein will expand under arterial pressure. A temporary access can also be placed and can last a couple months. These catheters can be place pretty easy, even if a patient is a difficult vascular patient. A skilled vascular surgeon will place a fistula where needed until a kidney becomes available. And if they don't qualify for a transplant they know their days are numbered anyway. In an emergency like that we don't look years ahead, we look at the now. Stop the emergency situation and correct what is needed once stable.

    Ok, for the second time, the main idea of my post was that the patient should discuss with their physician on the best way to stop the bleeding while TRYING to preserving the fistula if possible, in an emergency situation, not that we should let him bleed out.. Last time I will state this... Jesus!

    They actually don't recommend any CPR masks be used, and compressions only be done, unless it is a close family member. They have come to the realization that even mouth to mouth using a mask does not fully protect the one doing the rescue breathing. Most adult cardiac arrest is caused by a cardiac problem, thus the patient still has an adequate supply of free oxygen in their blood to get them through until a BVM becomes available.

    Why would he need sterile gloves and sutures? Go to the urgent care or hospital if you need sutures... if you are out in the woods clean the wound, but I wouldn't recommend suturing a wound that hasn't been adequately cleaned out, that will do more harm than good!

    Right. I agree. But asking the physician during an emergency just isn't going to happen. The patient more than likly had bleeding issues prior to the emergency and should have notified them and asked then. During the emergency is not the time to be asking, it's time for action. I mean he went to the neighbors to call 911. Meaning he either has no means to call out and ask himself or other issues are also present with this patient. you assume this person is the perfect patient. I bet you this patient can't even list all of his medications or reasons for taking them. There are good well informed patients and others just do what they are told or do what they want.

    Usually when out with friends and family you'd be willing to perform full CPR. I agree on strangers though.

    I disagree on the sterile gloves and sutures. This is a first aid kit for in the bush, out shooting. If area is rural your not getting to urgent care or other medical facilities very quickly. They are temporary items to be able to get you to a medical facility. And sutures are great for many things other than just sewing up soft tissues. Once at a medical facility any wunds can be reopened, flushed, cleaned, and repaired. Antibiotics will then be given to prevent infection to affected area.

    Dude you gotta have sutures, I borrowed some skin glue, steri strips, surgi cell, sutures and whatnot from work. I have used them all at some point. On myself, friends, neighbor...no need for urgent care and sometimes can't get there

    I didn't mean to hijack the thread at all (not like I normally do... :)).

    I think his fistula is grafted. It looks like a giant vein. This is the 2nd time it's happened in a month. The first time, he was able to call me and I ran over to his house, helped him keep pressure on it while the ambulance got here. I met him at the hospital and they put a stitch in it then sent him on his merry way. He told me when I visited him Monday night at the hospital that four days after he got the stitch to close up that hole, his vascular surgeon removed it for some reason. It hadn't healed and that's where the blood came from.

    This time it was pouring about twice as fast. I watched our doorbell camera because it recorded everything and he basically slumped over on our bench. I'm a left-handed phone talker so I scooped him up with my right arm and sat him on the porch. I was kneeling behind him, with my right arm crossing his body and with his head resting against it. I had a puppy pad in that hand and was gripping his arm while was putting a shitload of pressure where I thought the hole was. I did slow the blood flow considerably. When my wife took the phone, I was using my left hand to help support his weight. I was afraid to lay him all the way down because I guess I was afraid I wouldn't be able to keep him awake. I didn't really have a way to lift his legs without letting go of his arm. I was a lot calmer than I thought, besides getting pissed about the ambulance taking so long. Again, it's nothing against you all, I have a ton of respect for all first responders - I was just running out of strength and felt like he was running out of time.

    I didn't realize it but my right arm had so much blood on it that the EMT asked if I'd like them to doctor it up because it looked like a wound.

    I lost a neighbor and good friend who had Muscular Dystrophy where we used to live. He basically had a heart attack and died in my arms. The first responders were able to bring him back but he died in rehab. Thinking about it now, it may be time to sell and move into the country where I don't have neighbors...

    Anyways, it turns out my neighbor who bled, didn't have a stroke. They gave him more blood yesterday and his dialysis. Last night he had a really bad fever and they think he had sepsis from the wound, put him on antibiotics and he was close to normal this morning.

    The Chamber of Commerce donated money so we could have somebody come in and clean up the blood that was all over his house, which was really cool.

    The neighbor is in surgery as we speak and they are working on his arm. I'm going to look into all of your suggestions and also look into taking a class - it's been a while. Hopefully it won't happen again but I thought that the first time he bled. I'm going to look into the CAT too.
