Correct, must rip apart the entire assembly to change the height.
Best video I've found so far on this shock install on the Tundra:

I used this one when adding shims to my other Tundra / ‘17 PRO.

I can’t find the torque specs I used four years ago. I’ll have to research as I did last time.

For a stock vehicle. The OP has added 1" to rear. the 5100 is made for a 1" lift to keep the same available wheel travel.

Yes, but 1" should not make a difference, especially if he's carrying any weight in the bed.

I see mixed reviews on 5100 ride quality on top setting.

It’s definitely stiffer if a stiffer ride bothers you do not go top setting. Mine are set at top setting and I prefer the stiffer ride.

In order of stiffness, starting at soft:
    - Stock shocks
    - B 4600
    - B 5100 bottom setting (stiffer than 4600 due to +0.87" spring preload)
    - B 5100 middle
    - B 5100 top

I have no experience with the shocks set at middle setting. When I installed them I set them at top setting. :notsure:

    Yep

However it appears comparisons are Top setting vs 4600 or black shocks.

I’d like to know thoughts, if any, from those who have tried both Top and middle setting.

Final followup! I got the front shocks installed yesterday at a local shop, added the back ones myself today. Also CB +1 shackles at the same time. Front 5100 set is sitting on the bottom perch, the "stock" setting although providing a very small lift (less than in inch.) I preserved the factory rake of +2" at the back.

Ride is a little more firm than the stock black shocks. I expect anyone running 4600 series would have about the same experience, or those with TRD Off Road OEM shocks.

So, I had a 2013 Tundra SR5 2wd with 40,000 mi on it and with black stock shocks. It felt like a land yacht, rolling around every curve and swaying with each lane change.
I changed to 4600s at 70,000 mi and wow, what a difference! No more swaying and it drove like how the truck was supposed to. It was slightly stiff, but forgiving. It did give the truck a slight lift, but I think the stock shocks were worn out. I only had a 1.5" level kit on front and it became level after the 4600s were put on.
We got a boat and towing 5,000lbs barely made a difference in height. I towed 2- 3500lb pallets of sod, twice, about 8000lbs with trailer and no problems.
I now have a 2018 Tundra Off Road with 24,000 mi and the yellow Bilstein Toyota shocks and I feel they are definitely softer than 4600s. When towing the boat, it's about the same, but on the highway and mild dirt roads feels a bit softer.
I plan on getting a level kit for the front and eventually 4600s when the stockers wear out. I have no experience with 5100s, so I can't comment on those at all.

Anyone know if anyone is currently selling any of the 4600s on here?

I'm replacing my worn out stock shocks with the 4600s. This isn't my daily driver and is an 11 year old truck with high mileage on it. These should do just fine for what I need it for and I'm looking forward to seeing a better ride quality over the worn stocks.
