Mine does it when it was cold and if it has set for a couple of days. I hope this is normal. If not I got an issue.

Yeah that’s pretty much exactly what I am experiencing even if it just sits for 12 hours

This is probably unrelated to your post and noise...

When I got this truck I was amazed at the super fast start idle. My old school belief is that this is not particularly good for an engine because some parts are not immediately receiving oil. It’s not a lot, but overtime, there is more wear.

I have wondered about this and admit I get paranoid about no pre-oiling feature with such a massive engine....maybe this is why the oil viscosity is so low? I always let this truck run at idle and let settle down a bit longer than any other vehicle I have owned because of this. I think it’s probably an antiquated quirk, but I still hold back until I am sure the engine is warm and the OP is maxed out.

On the other hand, I remind myself that they go for hundreds of thousands of miles. Go figure?

I'm having same issue. Its loud at first and then settles down. It kinda sounds like its rubbing or grinding at a certain rotational point. I took to toyota dealer and they say its the power steering pump. I would think that the steering would be tight or I would have a leak, something.

The 5.7 v8’s tend to be noisy. I have heard people calling it the tick, tick, ticking of reliability. As long as your oil level is good and the noise is not excessive, I wouldn’t sweat it.

I thought maybe it sounded more like a timing chain issue due to pulley tensioners. I'll see after the power steering pump is replaced if that helps

Its your timing chain tensioners loosing their prime after sitting , as soon as you start the engine it takes a second for the tensioners to pump up with oil pressure . I see it alot in my shop with older second gen 5.7's . keep your eye on it , it will get worse with time . will need to replace the tensioners sooner than later .

Has there been any issues with 3rd generation 5.7 flex fuel engines? Also what kind of ballpark cost is this to get fixed?

Timing chain tensioner job is a fairly involved job , definitely not cheap , have to pull the timing cover etc..., over a grand usually .

There is no difference between flex fuel engines other than ECU programming and maybe a couple sensors . so all 5.7s are susceptible to it . However , in my experience its usually the engines that have seen a lack of oil changes or cheap quality oil that it happens to most . IMO I would not follow toyota's oil change intervals . I suggest full synthetic be changed every 5000 miles or 7500 kms .

The toyota dealer swears that it was the power steering pump. That wasn't it now they are saying the alternator. I'm concerned that I haven't had a problem with my steering at all or a problem out of my alternator. Think I'm going to take it elsewhere and see what they come up with. Thank you for your response.

Toyota has had a high idle for 30 years. I don’t think they have excessive wear if the oil is changed regularly. My grandma's 94 Avalon did the high idle. It is annoying if you’re like me and won’t put it drive till it at least drops below 1000

We do the same thing. I let it idle down year round.

If you switch from 0w20 to 5w30 it quiets down.

My truck seems to almost have two stages at high idle. When you first start it, it really roars for a while then quiets down but is still idling pretty high (certainly over 1000 rpm). I'm not 100% sure but I believe the tach reads the same in both stages.

I'm pretty sure it's normal as it's always done it and I read of at least 1 Lexus that does the same. However, I was curious what your experience with this is.

Elevated idle for cat-heating to reduce emissions.

    Back to the OPs issues,
Could be an issue with your cam adjuster not having proper pressure right off cold start.

Ive had a 2019 and now the 2020.
Same thing periodically, more so if it has not been driven for awhile.
Sporadic but both seem to rattle for a few seconds and I believe its from defrost/ac components.

I dont think its much to worry about given back to back new trucks doing the same thing. Fwiw
