Why do all the cigarettes lighter all turn off when the truck is off, wish one would stay on when truck is off. How hard would it be to wire it direct

If you want a full time 12 outlet you could always wire one to your battery. I think they just wire them like this so you won't drain your battery by charging something for too long. I have thought of running a full time outlet but haven't needed to yet.
There are a few ways to do it, simplest is just run a fused power wire from your battery to a new outlet that you could put anywhere you want. You could also wire in a switch in case you wanna turn it off, or a relay run to some kind of accessory for remote turn on. Lots of options.

None in the tundra are hot all the time. I have added a few to my truck but didn't wire them hot. It should be easy enough and by going aftermarket you have lots of options for different style outlets. Some day I'll add a hot to mine, probably a dual quick charge usb outlet, I have one laying around that I should use, or sell lol
