P0156 and P0158 error codes

    Hi folks, I see the OBD codes list on here and have also poked around the net but didn't come up with anything specific to both thee codes. Tried a new bank 2, sensor 2 already but it came back. From what I read it could be bad wires, hole in exhaust, bad fuel/air sensor or bad computer.

    Anyone else had this combo of errors?

    I just got it a few weeks ago- the error codes were there when I got it. Truck has 207k km, so about 130k miles.

    My Torque app confirmed the high voltage...

    Ni, I will try that and report back. I will also have my first tank of fuel gone soon so I can report back on mileage.

    I read up a bit on the MAF and supposedly the K&N filter (came with the truck) can make them dirty.. Do you guys recommend swapping that for a regular filter with no oil?

    I have cleaned the MAF sensor with the appropriate cleaner (thanks!). The infinitely wise internet tells me it will take a few key cycles or possibly an OBD II cycle before I will see a difference in my computer and sensor readings.

    Only 12mpg though a short haul with my trailer and mostly city driving..still seems a bit low. Cleaned MAF but same error code came back. The 0158 is up now. 0156 hasn't come yet, but last time it came shortly after.

    Still struggling with this. Took it to the dealer. They said my wiring was fine but the new sensor was "lazy". So I ordered the factory part. Put it in today and already have a P0158 code pending. No CEL yet but soon, I'm sure.

    It sounds like the 02 sensor and its affiliated wiring may not be the issue. Since it is reading high voltage that means that it is running rich. I would make sure that the air intake portion does not have anything blocking it (dirty air filter, debris sucked into the intake, etc. The throttle position sensor could be on it way out.

    My guess is that one of the fuel injectors is sticking, causing excessive fuel to be dumped into one or more of the cylinders. Are you getting any misfires or rough idle? If so I would check the spark plugs/wires.

    If you make sure that there is no debris and a clean air filter in the intake. Then ascertain that there are no misfires/rough idle. Then I would proceed to run a fuel system cleaner through it next, if the issue still comes up, I would clean the fuel injector and see what the flow is like.

    If all is well with the injectors, then it could be your TPS, or your ECM (might need a flash or a new one). It could also be your engine coolant temperature sensor, you should be able to get a scan tool out and watch the live feed to see if it is reflecting temp changes like it should.

    Thanks for all that. Some of it I can do, others I would not be comfortable with. I will do the easy ones ASAP. I do have a new air filter but didn't check the intake otherwise....

    Sooooo. I know it's not a tundra or a Toyota at that but I had an Isuzu trooper that had a series of problem because one little part failed. I kept getting bank 2 sensor 2 fail o2. And random misfire codes. Well after deep research and different parts I found the culprit.
    Pcv valve failed, causing oil to just dump back into the intake ( that motor was know for bad rings didn't help lol)
    Well since now I'm burning a bunch of oil and not all of it is being burnt out I ended up clogging and failing egr valve. Now since egr isn't working right I started getting having random misfires since egr can't regulate am out of exhaust back into intake for emissions causing random cylinders not to combust right. Now since I'm just basic running oil and exhaust in my intake system not being combusted I ended fail an o2 sensor.

    End of story new pcv valve egr valve and cleaning of sensor motor ran great.

    Thanks,I will check those items if possible too. Right now it runs like a dream- very smooth and no misfires, oil nice and clean. But I will check nonetheless.

    Ha yes- it kind of ain't broke bit the CEL is on and my gas mileage seems a bit low...

    Hi folks, updates and questions...
    I checked the pcv valve. It was filthy but operating so I cleaned it and stuck it back in. Should I replace it just because?

    Also checked the intake- nothing blocking. Air filter is new but what is this permanent looking screen above? It appears to be attached to the top of the airbox and I am not sure if it should be replaced periodically? Hard to tell if it is clean and I am still worried about oil coming off the kn filter causing issues (hence going back to stock air filter).

    Oh and I cleaned the throttle body.. I also swapped the placement of the two post -cat O2 sensors and went for a drive. The P0158 code is pending again already on bank 2 sensor 2. This confirms my sensors are both fine and there is a problem on bank 2 somewhere up the line. Where is the question. Still no misfires or rough running.

    I guess I will have to bring it in again and have them check it out, including injectors, TPS and ECM and engine coolant temp sensor as suggested.

    Kenomouth64- it just occurred to me that when you said to test the injectors that need not mean pulling them off and doing it manually, but that I may be able to use my torque pro app and obd2 scanner. I am not used to such technology.

    Has anyone done this? I see options for fuel trim sensors on each cylinder and also short term and long term fuel trim on each bank. Will these tell me if the injectors are OK?

    The fuel trim on each sensor was not available when I turned the car on. The trim on each bank is, and here is the graph and some info. Does it mean anything??

    Before you go chasing your tail. The code your are getting is it random miss fire? Because if there was just one injector/ cylinder not firing you could tell real quick. Sounds to me like something is faulting out your bank 2. Or you have bad wiring. I Would check you EGR valve and make sure all your wiring is good. Not cut, shorted, plugged in.

    Thanks- no misfire codes, only o2 high voltage. I had the Toyota dealer check the wiring although I have little faith in them after doing so. I did check for voltage on the heater circuit and it's about 12v key on/ignition off and jumped to 13.8 when the truck was on.

    The sensor Input voltage was good to start but jumped around when I tried to read it and confirmed the "high voltage" when running.

    I didn't check the wiring at the ECU end because I am worried about mucking around with it... And I really don't know where to look to be honest.

    I will add that I have using my scanner a bit and observed some low short term fuel trims when cold- about 17-19% which seems to be too low. This is happening on both banks but a little more so on bank 2 where the error code is. Seems to be a sign that I am running too rich for a reason the ECU has not identified.

    Well, after many hours and many dollars my local dealer finally fixed this issue. The air/fuel ratio sensor (bank 1 sensor 1) replacement solved it. No error codes related to that sensor ever came up.

    They are still baffled, but it's fixed!

    To bad I didn't see this early, would of said replace sensor 1 and 2 on bank one which is cylinder 1 side, driver's side. nothing baffling about this, especially if you can watch the cross counts of the O2, then would just replace the bad one. Sensor 1 could be weak, making sensor 2 look bad, or 2 could be bad......just replace both, they do not last forever......and I will say that again, they do not last forever. BTW, OBD2 codes are pretty much the same with all vehicles.
