There was someone on here a few months ago looking for that same part. May have some luck searching around on the forums for the thread/post about it.

Man, I never noticed how much chrome mirrors stick out like a sore thumb on a black truck ...

@Linguine252 here's the exploded parts for an '03 access cab: https://parts.toyota.com/a/Toyota_2...58/FRONT-BUMPER--BUMPER-STAY/841420-5252.html

If I'm reading the FULL SIZE version of that image, the part number is 521150C020 (formerly 52115-0C020) which would be around $20 if you order from parts.toyota.com for pickup from a local dealership.

The diagram makes it appear as there's another trim piece in front of it which you may also be missing, you would need the black version (not the grey which comes on lighter vehicles) which is part number 5254134020C1 (formerly 52541-34020-C1)

I can't make sense of the hardware holding it on, lookes like it may only take one fastener, with one nut? Huh.
Fastener is p/n 52491-35010 and its nut looks like 90080-17236 to me. I may be wrong on any of this, either confirm my research by hitting the link above, or call your local dealership.

Just know, if you order direct from the dealership parts counter vs. using the parts.toyota.com website, you may pay anywhere from 10%-60% more. Toyota gives you direct incentives for beginning your order through their website. You won't see the MSRP discount offered until you 'add to cart' and choose a dealership to pickup or ship from. I know it sounds stupid for them to do this, but it's actually the same for other vehicle brands too....
