When all was said and done it fit okay but I am wondering how long it will last with this kind of damage. If it wasn't 5ºF outside and I wasn't working in the snow on a truck I really needed to work I would have returned the radiator. As it is, I had no real choice but to use it as is.

Denso 221-0517 Radiator.
This radiator was not an exact fit, as others have mentioned. I had to use the original side mounting brackets from the old radiator and I also had to elongate the bottom holes. The Denso 221-0517 Radiator is about ¼ inch longer than the original which prevents the holes from lining up on the fan shroud.
Also my original order was delayed by two weeks, place a new order with quicker delivery, but both orders shipped at the same time. Both of these packages were opened and taped shut before delivery to me. Looked somewhat suspicious, but the radiators inside looked okay.

I waited for a warm (I live in northern Alberta, and it is winter) day, and pulled the front of the truck into the garage and onto a garage mat to change the radiator over. I pulled the skidplate (bottom engine cover) off, removed the 4 shroud bolts and moved the clamps on the hoses where the hoses met the radiator. I carefully removed the top hose from radiator, and set a clean pail underneath to collect fluid. After a while, I did the same for the bottom hose. I did the same for the automatic transmission hoses, plugging the hoses with the caps from pens. Finally I removed the 4 bolts holding the old transmission to the truck grill.
A little patient tugging and wiggling got out the radiator.
Old radiator had slightly more secure/meatier nuts for connection to the grill and for the shroud mounting. But, it was not difficult to install the new radiator. Put back most of the collected coolant (3.5 litres?) into the radiator, rest into the overflow, and top off the overflow. No leaks after 1 day, put back the skidplate. I lost maybe 1.5 cups of coolant, and a couple of tablespoons of transmission fluid.
Only minor mechanical skills needed, but patience is needed in removing or installing the radiator.
The Denso radiator looks every bit as good as the original radiator, which had a very tiny leak.
